11/28/2023 EnchantaVaughn Approved Behavior: Client struggled to display his anger appropriately to day with excessive horseplay as he self reported he was in a positive mood but then became irritable as QMHP wanted to discuss his verbal and physically aggressive behaviors at home, school and community. Client then reported a verbal altercation with a peer 1xâs with moderate intensity for 30 seconds at school today. Behavior: Client then reported an incident today in which he and a peer were horse playing and and peer got angry and punched him 1xâs with moderate intensity in the face during the 30 second incident. He reported he then contacted QMHP from his cell phone in the bathroom to make him aware of the situation. Intervention: QMHP probed client as to his request for todayâs individual counseling session. QMHP approached client today to make client aware of the recent conflicts he has encountered in the last few weeks in the home, school and community. QMHP advised client that he needs to know how to respond to his peers as well as to know when they aren't in the mood to horseplay. Intervention: QMHP then proceeded to encourage client on reducing the acts of horseplay, which in turn will reduce the chance of physical altercations and conflicts by excessive horseplay. Response: Client seemed receptive to todayâs individual counseling session in which, he verbalized understanding of session. Client verbalized that he is aware that his behavior is in need of improvements, and vows to improve it with the help of QMHP. Client verbalized that he likes to engage in acts of horseplay and at times others get angry when there want to fight him when the horseplay becomes too aggressive. Client stated that he knew the consequences but didnât mean to spit at the peer when angered. Client stated he is aware that his behavior is in need of improvement, he stated he will do his best to improve. Client stated the negative behaviors he displayed today and reported the teacher was wrong for calling his mother when he didnât start the altercation. Response: Client verbalized that he is aware that his behavior is in need of improvements, and vows to improve them but reported others also need to learn to adjust to his attitude as well. He responded positively to positive reinforcement to client for his participation and positive thinking and wanting to reduce incidents of aggression directed at the peer today during the horseplay incident. QMHP will continue to work with client on decreasing all maladaptive behaviors at home, school and community. QMHP has scheduled the next counseling session in which he will educate client on exercising self-control and assist him with gaining attention in appropriate and constructive manners.
